HISTORY

Post-Acute Network Solutions partners with Chicago-based managed care plans, senior residential facilities and medical providers to provide an innovative care coordination model that enhances overall member and family satisfaction, improves quality and controls total costs.

Formed in 2012, Post Acute Network Solutions helps select managed care health plans provide in-person care coordination services and delegated utilization management for moderate- and high-risk, dual eligible beneficiaries who receive Medicaid long-term services and support (LTSS) or home and community-based (HCBS) waiver services.

Prior to Post Acute Network Solutions (PANS) and over the past 30 years, Brian has served as a leader of the post acute industry.  He has owned and operated a variety of senior housing communities, transitional care and skilled nursing facilities as well as supportive and independent living senior housing.  He is also a partner in ancillary care businesses including home health, rehabilitation, hospice, and medication management.

Brian is also the Founder and former President of the Affordable Assisted Living Coalition and the former Chairman of the Illinois Department of Professional Regulation Nursing Home Administrator licensing board and a well respected speaker on senior housing and transitional care at national conferences.

Randy has over 30 years of operational experience in developing and working with insurance companies, physician organizations and health care delivery systems specifically in the areas of commercial and Medicare managed care risk products. These business models have all dealt directly with agreements that were modeled under the assumption of financial risk or a percentage of premium contract.

Prior to working with PANS, Randy was the President and CEO of HealthSpring of Illinois, a Medicare Advantage Prescription Drug (MAPD) plan, operational throughout Chicago, Illinois. During the first eight years of operation, Randy guided the organization through all phases of development, sales and operation, helping it grow to be the second largest MAPD in Chicago. Approximately twenty (20%) of the overall HealthSpring membership was considered to be dual eligible, and the Illinois plan was a recognized leader in reducing unnecessary admissions and re-admissions while improving the overall quality of care as measured by STARs.

In addition to having worked with other multi-line health plans (Blue Cross, Blue Shield, Sanus/New York Life), Randy has considerable senior level experience working with physician-governed organizations (PhyCor, NAMM, WellMed), where he provided executive leadership in all areas related to operational and delegated responsibility, including utilization/quality oversight, overall network operations and organizational fiscal integrity.

With over 30 years of experience in healthcare, including medical device development and manufacturing, post acute provider businesses for seniors, physician services and Medicare and Medicaid managed care insurance companies, Craig is a principal in Post Acute Network Solutions and is involved in all areas of operations.

Craig was previously CEO of Home Physicians, a Chicago-based healthcare service business that provides home-based primary care and podiatry physician services for over 15,000 Medicare patients in Chicago and Baltimore.  He also developed their Medicare Advantage services for comprehensive assessments and chronic care management.

In addition Craig has held sales, marketing, management and leadership positions with American Hospital Supply Corporation, Baxter Healthcare, Allegiance Healthcare and Cardinal Health.

Kathleen is a health care leader with over thirty years of experience. She has a broad clinical background with twenty years of case management experience, including eighteen years in case management leadership positions. Prior to joining PANS, she was a manager with The Camden Group, a healthcare consulting company, where she worked to develop the company’s case management program and functioned as interim director of case management for hospital clients across the country. In addition, she has experience managing case management departments for Medicare and Medicaid managed care organizations, home care consulting, and disease management. She is a certified case management administrator and a member of the Case Management Society of America and the American Case Management Association. She frequently speaks and writes about case management and other areas of expertise.
